What to Expect in Your First Year with Breast Implants

By Dr. Drew Davis

It’s essential to recognize that although the surgery is a one-day procedure, the exciting final results unfold gradually over several months. Knowing this evolution and what is considered normal will help you feel confident and prepared throughout the entire process.

Sacramento plastic surgeon Dr. Drew Davis provides a month-by-month overview of what most patients experience during their first year.

Weeks 1–2: The Immediate Recovery Phase

The earliest days bring the most noticeable changes. Swelling, tightness and temporary asymmetry are completely normal and expected. Your muscles and tissues are adjusting, so things may look and feel higher or firmer than anticipated.

Activity is limited during this stage. Gentle walking is encouraged, but lifting and strenuous movement are off-limits. You’ll wear support garments as directed, and incision care is essential for proper healing. This phase requires patience, rest and realistic expectations.

Weeks 3–6: Early Healing

By weeks three to six, swelling begins to ease. The implants still sit higher on the chest and may feel firm, but early improvements are noticeable. Many patients return to work, drive and resume light daily activities.

Some mild soreness or sensitivity is normal as tissues stretch and adapt around the implants. You’ll begin to feel a reassuring sense of normalcy during this stage, even as you continue your progress toward your final results.

Months 2–3: Beginning of the “Drop and Fluff”

Around the two- to three-month mark, most women notice their implants softening and settling into a more natural position. This is the start of the “drop and fluff” process, when implants move lower into the breast pocket and create improved cleavage and shape.

Even though the outer appearance looks better, internal healing continues. It’s a period of steady, subtle changes that make a big impact on your overall result.

Months 3–6: Results Refine

Between months three and six, your breasts typically look and feel much more natural. Symmetry improves, swelling continues to fade and you get a clearer sense of your long-term shape.

With consistent care, scars will soften and begin to lighten. At this stage, many patients feel comfortable wearing fitted tops, swimwear, and other more revealing clothing.

Months 6–9: Approaching the Long-Term Look

Most swelling has resolved by month six, and the implants sit closer to their final position. The chest feels more flexible, the skin less tight and the implants more integrated with your natural tissue.

Any lingering sensitivity usually fades during this time, making everyday movement feel easier and more natural. This is also around the time we usually have patients back to the office for a check in visit and to take “after” images.

Months 9–12: Your Final Results

By the one-year mark, breast implants have fully settled into their long-term shape and softness. The color and texture of scars noticeably improve as they mature. However, the final appearance may take a full year or more to be fully realized.

Our patients are able to enjoy stable, predictable results and confidently resume all exercise, travel and lifestyle routines by this time.

Thinking About Breast Implants in 2026?

While the immediate results of a breast augmentation are quite noticeable, in reality this procedure can best be thought of as a year-long journey. Each phase contributes to the final look and feel, and knowing the timeline helps set realistic expectations and ensures a smoother, more confident experience.
