Upon check-in for your consultation, a signature will be collected to acknowledge that you reviewed both the HIPAA Acknowledgement Form and Financial Policy and are aware of our policies. The other forms are provided for your convenience.

Please note that we will need to collect a $100 consultation fee over the phone with a credit card at the time of scheduling your consultation. This fee will be applied to the overall cost of your procedure, should you decide to move forward.
