During Your Consultation
During your breast implant revision consultation, Dr. Davis will assess the shape, volume, and symmetry of both breasts and take measurements. He will also asses if you have any signs of a capsular contracture or implant malposition. You will have an opportunity to discuss your goals and ideal result. You can also share any specific concerns and have all of your questions answered. If possible, it is very helpful to bring with you any records of prior surgeries and your current breast implant information (size, style, fill volume if saline).

Technique
Depending on what each individual patient needs to get the results that she wants, various different techniques are utilized. During surgery, Dr. Davis makes every effort to use the previous incision sites to reduce additional scarring, but there are times where additional incisions are necessary. Common parts of implant revisions include changing implant size or switching from saline to silicone, adjusting implant position, removing scar tissue called capsular contracture, or completely removing the implants. It is very common to perform a lift with implant revision surgery. Dr. Davis will tailor the plan to you depending on your starting point and goals.
Recovery
After surgery, you will be placed in a supportive bra and will return to the office the next day for us to check on you. Sometimes drains are needed as part of the healing process to make sure fluid does not collect around the new implant. We will continue to check on you as you recover, and are of course here and available any day if you have any questions or concerns. Recovery from a breast revision varies depending on what you had done. This will be thoroughly explained to you at both your consultation and pre-operative appointment.

Q&A with Dr. Davis
It is best to wait 6 months after prior breast surgery before having a revision. This allows the body adequate time to heal and the implants to settle so that an accurate assessment can be made as to what changes are needed.
Women seek out a revision of prior breast surgery for many reasons. Sometimes just small touch-ups are needed to adjust implant position and symmetry or increase or decrease implant size. Other times more involved surgery is necessary because of changes from pregnancy, weight fluctuation, capsular contracture, and implant rupture.
Mesh is a medical product that reinforces and supports tissue while it heals. Originally it was used for hernia repairs but now has widespread applications and is becoming more commonly used in breast lifts, revisions, and reconstruction. Some are entirely synthetic, and others are derived from bovine and porcine dermis. All are treated so that there are no living cells, only the scaffold, or framework, of skin and they are incorporated into your body over time. I use these only when indicated, but when they are needed, they can make all the difference in a successful result.
These are the classic symptoms of a capsular contracture where the thin scar tissue that normally forms around an implant becomes thicker and can squeeze the implant. This can cause the implant to change shape and also to move up on the chest. For some women, it can also be painful. This can be fixed with surgery to remove the old implant and thickened scar tissue and place a new implant. Sometimes, mesh is needed for extra support for the new implant.
Over time, implants can fall more into the armpit and this can be for several reasons. When a woman's rib cage is more rounded, implants tend to sit more laterally and can feel like they are more in the armpit. Sometimes, the implants are too wide for a woman's frame. And sometimes, gravity and time cause the pockets to enlarge giving the implant more room to move around. Surgery can be done to change implant width and also to tighten up the pocket to help keep the implant where you want it.
